PLEASE READ IT CAREFULLY. Date: I recognise that Scott Langston will collect certain personal information about me. This personal information, or personal data, is anything that could be used to identify me as an individual. I recognise that Scott Langston needs to see my personal information to assess whether I am able to access counselling with him. I accept that this helps guide both the assessment process, and the decision-making during counselling. I have been informed that The DPA and the General Data Protection Regulation (GDPR) require that all organisations that store personal information about people may only do so provided that the information is: processed lawfully and transparently; collected for specific legitimate purposes; limited to what is necessary; accurate and up to date; kept for no longer than is necessary; and processed in a manner that ensures appropriate security. I note that Scott will collect my personal information via the website: www.expatcounsellingnow.com, over the video meetings, in chats and in writing via email. I have been informed that Scott will store my personal information electronically. Personal information is stored electronically on devices that are password protected, and in files that are further password protected and only accessible by him. No paper records are kept. According to the GDPR, this personal information should be stored for no longer than is necessary, and Scott will usually store my information for up to 5 years. I accept that Scott will ask me via the Internet for to provide him with: my name, telephone number/contact details, availability, the psychological issues that I would like to address, and any symptoms I may be experiencing. Scott will also collect further information from me that may include: my goals, previous therapy I might have had, current medication, network of support, my employment circumstances and general financial health, physical health issues, alcohol and drug use, appetite and sleep, family structure, details of my family situation. I have read and agree to the Limits of Confidentiality posted on the ExpatCounsellingNow website and provided to me by email. I agree I recognise that I can request a copy of any information being stored with ExpatCounsellingNow and that this will be provided to me within one calendar month of my request. I can also request that all information about me is deleted. I must do this in writing via email and Scott Langston undertakes to erase my records within one month of my request .
